 Receiving Accommodations Minimize

In order to receive accommodations for a disability, the student must pick up an Application for Disability Services in the ACCESS Center or he or she can click here for an online form which can be completed and mailed to the college. This form asks for basic background information, as well as important information about how the disability will impact the student's functioning in the classroom. The student will meet with the Director of Disabiity Services, Cindy Yarberry, to discuss this form and the requests for accommodations. It is the student's responsibility to gather the documentation necessary to support the particular request. Acceptable documentation may include a psychoeducational assessment, a letter from a doctor, an audiogram, etc. The Director will specify the type of documentation needed. After the documentation is in place, the student will receive a letter of accommodation to show each instructor. It is a good idea for the student to discuss the accommodation with the instructor so that they are both clear about its implementation. Problems with implementing accommodations should be reported to the Director of Disability Services.
